As a Case Manager in our Community Connections Program, you'll engage with vulnerable individuals experiencing homelessness or insecure housing. Join our dedicated team to provide vital support and assistance.

**Duties and Responsibilities**:

- Conduct assertive outreach to engage with homeless individuals
- Screen referrals and assess eligibility for the program
- Provide case management and link clients to support services
- Advocate for clients at individual and systemic levels
- Maintain accurate client records and participate in networking activities

**Requirements and Attributes**:
- Tertiary qualification in Social Work or equivalent
- Experience in outreach services and complex case management
- Understanding of social determinants of health and homelessness
- Excellent communication and interpersonal skills
- Unrestricted Victorian Driver's License
