**About the role**
We're looking for a Social Worker (Team Leader) to join our Inpatient Rehabilitation team at.8 EFT. The primary role of the Social Worker within the Inpatient Rehabilitation Program (IRP) is to assist in a smooth transition from the hospital to home. Social Workers take a holistic approach when working with consumers and their families to identify any areas of concern for discharge. If appropriate the social worker will assist with:

- Various referrals to community services (such as the Aged Care Assessment Team)
- Providing information for post hospital services
- Short term counselling
- Patient empowerment and advocacy
- Co-ordination of family meetings

The Social Work team provides patients with additional support to improve psychological and social functioning and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. We are a dynamic and innovative group who is always willing to share knowledge and peer support.

**Key Responsibilities**
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Participate and fulfil requirements of allied health and discipline specific orientation, induction, mentorship, supervision and performance management programs

**Skills and experience**
- Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree)
- Demonstrated ability to monitor, evaluate and modify own performance through reflective practice
