**About the role**

An enthusiastic Social Worker (Graduates welcome to apply) is sought to provide services to various clinical areas at Ballarat Health services. This position aims to provide extensive learning and development opportunities across various clinical programs. Primary responsibilities include clinical skills and a passion for working in healthcare, excellent communication and collaboration in pursuit of team and patient/family goals is essential. Sound knowledge and experience in undertaking psychosocial assessments, excellent communications skills, complex discharge planning and the ability to work well under pressure and in pursuit of patient and team needs/goals is required. You may be required to work in various areas across the health service, Acute, Women's and Children’s, Oncology, Inpatient Rehab and Aged Care. This is an exciting career opportunity.

**Key Responsibilities**
- Provide assessment, diagnosis, treatment / intervention and advocacy for BHS clients presenting with a broad range of conditions requiring inpatient services.
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Ensure the ongoing ability to provide clinical excellence

**Skills and experience**
- Ability to adapt, be flexible and learn quickly
- Ability to work within a fast paced environment whilst maintaining a professional manner
- Ability to plan and problem solve
- Demonstrated sound knowledge in Social Work theory and practice skills
- Some knowledge of working in acute/sub acute environments (via clinical placement would be desirable)
