**About the role**

As a Grade 2 Social Worker, working in the Geriatric Evaluation Program (GEM) you will be responsible for providing comprehensive Social Work input and best practice patient centred care, with the aim of: improving patient outcomes, achieving maximal operational effectiveness and facilitating sustainable discharge plans for patients admitted to this program.

**Key Responsibilities**
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Participate and fulfil requirements of allied health and discipline specific orientation, induction, mentorship, supervision and performance management programs
- Provide patients with additional support to improve psychological and social functioning
- Ensure each patients social, emotional and cultural needs are met
- Minimise the negative impacts of illness and hospitalisation on patients

**Skills and experience**
- Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree)
- Demonstrated ability to monitor, evaluate and modify own performance through reflective practice
