**ROLE OVERVIEW**:
**About the Role**

Seasons Living Caloundra community is seeking a skilled, passionate and collaborative Care Coordinator to join their team. The Care Coordinator is responsible for managing residents’ services and care through case management, staff allocation and rostering; ensuring appropriate support services are responsive to the individuals needs and relevant industry standards, while meeting all performance indicators.

**Core Responsibilities**

Management:

- Manage and achieve agreed budget targets and key performance indicators.
- Manage human resources in a fair, equitable and financially responsible manner.
- Collaborate with Community and/or Care Manager on issues related to service provision.
- Successfully recruit and onboard care staff, ensuring a positive employee experience.
- Monitor training and education of staff to ensure development of skills and compliance.
- Provide support and supervision to staff, ensuring delivery of their role.
- Rostering of staff and scheduling of resident care services to meet required care needs.
- Maintain documentation consistent with appropriate standards; ensure billings are correct.
- Manage, review, and put correct methods in place for all care related incidents.
- Ensure care meets accreditation and quality standards, including disaster response planning.
- Management of and participation in on-call roster as required.

Resident Care:

- Consult with residents and their significant others to design, monitor, review, and coordinate care plans following comprehensive care assessment.
- Consult with Registered Nurse on care matters; engaging clinical nursing expertise to assess, plan, monitor and review complex clinical care elements as required.
- Coordinate care services and ensure they are correctly implemented.
- Ensure appropriate handling of medications by care staff, in a community care environment.
- Coordinate service interface with external clinical professionals (General Practitioners, Medical Specialist, Hospitals, Nurses, and Allied Health)
- Maintain comprehensive confidential client files, consistent with legislative requirements.

- Demonstrated knowledge of the DVA Community Nursing Program, Home Care Packages, Aged Care Quality Standards and Client Rights and Responsibilities.

- First Aid and CPR Certificate
- Current Police check
- Covid-19 vaccine evidence
- Unrestricted Australian working rights
- Diploma or Certificate in Community Services or equivalent discipline
