The Team Leader will lead the day to day clinical and administrative operation of the Blue Mountains Assertive Community Treatment Team (ACTT).

The Blue Mountains ACTT provides a range of clinical and psychosocial interventions for consumers with persistent mental disorders who require an enhanced level of support to sustain community living. The ACT Team is able to make more frequent and assertive efforts to engage consumers in their home settings to stabilise their health, help them identify and achieve their social and vocational goals and attain greater autonomy.

The ACT Team provides in-reach to inpatient settings; assisting in the allocation of care coordination services to consumers that commence transition to the community following extended inpatient admissions. The team offers a care coordination service to all allocated consumers including to consumers on community treatment orders.

**What you will bring to the role**
- Tertiary qualifications, professional registration/membership as follow, Nursing, Occupational, Social Worker, Senior Psychologist and Clinical Psychology.
- Advanced clinical experience, knowledge and skills in community mental health service provision and experience in health service management.
- Demonstrated change management skills
- Knowledge and understanding of National and State directions for mental health services, demonstrated commitment to evidence-based practice
- Demonstrated leadership skills including implementing evidence-based practices, undertaking service planning and service evaluation.
- Demonstrated superior written and verbal communication, time management and computing, skills; ability to work collaboratively and independently
- Understanding of and commitment to recovery-oriented services and consumer involvement in service design, implementation and evaluation.
- Current Driver's License and willingness to use for work purpose travel.
