**About the Role**
Uniting Communities are seeking a Care Team Coordinator to support and lead our team of care workers in providing best practice care for our customers living independently in their own homes. Your ability to show enthusiasm, positivity, and empathy as you work with the care team, customers and their families is essential to our service. To achieve exceptional outcomes you will need to be a team player yet able to work autonomously, be resourceful, manage your time well, be a good listener as well as problem-solve. In this dynamic and fast-paced role, no two days are the same.
This position will support our Home & Community Care Workers based in the South of Adelaide (Fleurieu Peninsula, Victor Harbor, Adelaide Hills, Murray Bridge)
Please note as part of this role you will be required to work weekends and public holidays on a rotating roster.
**Your responsibilities include**:
- Develop and maintain strong relationships with customers and care workers
- Promote a strong customer focused culture
- Overseeing and implementing quality improvements to support high quality caring
- On-boarding, mentoring and supervision of care workers
- Actively participate in the end-to-end recruitment process and strategy
- Ensure all documentation, procedures and reporting meets relevant guidelines and legislation
- Maintain and encourage a safe work environment in accordance with the Occupational Health and Safety legislation
- Diploma in Enrolled Nursing (desirable but not essential)
- Have exceptional customer service skills
- Time management skill and attention to detail
- Have strong administrative and computer skills using information management systems
- Have the ability to work in a team and build positive working relationships
- Have an understanding and experience assisting clients within aged care sector (ideally)
- Have experience in coordinating services in a fast-paced environment, with competing priorities
