About the role
This position will provide high quality mental health assessment, formulation, treatment planning, therapy, case management and service coordination to adults who have severe and enduring mental illness and whose needs cannot be met by other primary health and community based service providers.
The Ipswich Rural Community Care Team (CCT) provides medical and case management interventions for consumers within the identified geographical boundaries of West Moreton Health. Ipswich Rural CCT is a multidisciplinary team providing specialised mental health interventions in a culturally diverse population to enhance community integration and networking with available support agencies.
About you
- Demonstrated ability to provide consumer focussed discipline specific as well as interdisciplinary clinical services including assessment, planning, intervention and evaluation.
- Sound knowledge of or the ability to acquire recovery principles in a manner consistent with social work values and practice standards in the context of a community mental health setting.
- Demonstrated ability to maintain respect for others, remaining flexible, fair and willing to compromise when dealing with team members.
- Demonstrated ability to articulate a social work perspective and the priorities for assessment and intervention in the context of the multidisciplinary team.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
