**About the role..**:
Exciting opportunity for a talented **Transition Coordinator - Accommodation** to join our passionate Disability Services Team.
In partnership with the Operations Team, you will be responsible for the end-to-end coordination and smooth transition of new participants into their Supported Independent Living home. Demonstrating a commitment to the delivery of seamless innovative, high-quality services to participants and stakeholders. The Transition Coordinator will nurture relationships, ensuring the participants accommodation care needs, goals and preferences are addressed.
**Key responsibilities include**:
- Working with the Service Managers to develop the participants Roster of Care (ROC).
- Ensuring all new homes have the required equipment, resources and furniture required to create a safe and functioning home for residents when they move in.
- Developing, leading and facilitating the transition planning process, engaging with internal and external stakeholder to ensure a smooth transition for the participant moving into their new home.
- Ensuring participants support plans and risk assessments are in place, up to date, saved to the participants file and communicated to the operations team.
- Gathering all relevant and significant contact details for the participant, inclusive of emergency, health and wellbeing details. (eg doctor, chemist)
- Quality Management.

**Essential Criteria**:
- Relevant tertiary qualifications in human services, or social sciences/ or equivalent experience.
- Demonstrated experience and skills in improving and supporting the quality of life of people with disabilities as outlined in National Standards for Disability Services.
- Demonstrated experience in provision of Community Services.
- Demonstrated ability to work in partnership with participants and stakeholders in the provision of support to the person with a disability.
- Detailed knowledge of Person-Centred active support and procedures.
- Demonstrated high level communication and complaint handling skills.
- Demonstrated ability to manage systems of work in accordance with policies and procedures.
- Demonstrated ability to lead and work with a diverse team.
- Ability to chair and facilitate meetings with external and internal stakeholders capturing priorities and negotiating time frames.
- Exceptional organisational skills.
- Critical thinker with strong analytical capabilities.
- Current NSW Drivers License.
- Current First Aid Certificate.
