**About the role**
The NDIS Support Coordinator is responsible for providing NDIS participants with support in coordinating their Plans, budgets, and negotiating appropriate supports and services. They will ensure each participant’s Plan is implemented and their goals achieved.
They will establish a positive collaborative relationship with the participant and their support network, assisting them to identify, link and coordinate support within the local community, as well as build skills, overcome barriers and achieve goals.
- Provide advice and guidance to participants with various NDIS packages, building their capacity and ability to navigate service systems and developing resilience in the participant’s network
- Maintain an understanding of client needs and use this knowledge to plan and provide recommendations on the Disability Service
- Liaise with and report as required by NDIA in relation to NDIS participants and their Plans
- Navigate, influence and negotiate with public sector and community-based service systems regarding the delivery of appropriate supports in accordance with NDIS Plans
- Provide input into the systems to collect information required for reporting and planning
**About You**
- You have minimum 2 years’ experience in a similar role in community services or case management
- You have a Tertiary Qualification in Social Work, Health, Community Services or a relevant field (preferred but not essential)
- You have a strong understanding of the NDIS framework
- You have Experience building and maintaining professional, client-focused relationships with stakeholders
- You have the ability to build the capacity of NDIS participants and their support network; to understand and navigate service systems
