We are currently seeking a compassionate and skilled Social Worker to join our dynamic team.

**Key Responsibilities**:

- Conduct comprehensive assessments to understand the needs of clients and their families.
- Develop and implement tailored support plans that promote social inclusion and well-being.
- Provide counseling and emotional support to clients facing various challenges.
- Collaborate with multidisciplinary teams, including therapists, care coordinators, and community organizations to ensure holistic support.
- Advocate for the rights and interests of clients to enhance their quality of life.
- Facilitate workshops and support groups to empower clients and their families.
- Maintain accurate records and documentation in compliance with NDIS requirements.

**Qualifications**:

- At least a degree level of qualifications
- Experience working in a community setting, particularly within the NDIS framework, is highly desirable.
- Strong interpersonal and communication skills.
- A passion for supporting individuals with disabilities and their families.
- Ability to work collaboratively in a team environment and manage caseloads effectively.
