About the Role
The CN will work as part of a multidisciplinary team in a Primary Health Care / General Practice environment to create and implement an innovative community based mental health model of care for Aboriginal and Torres Strait Islander people. Mental health clinical services will include assessment, case management, collaborative care planning and therapeutic interventions within a recovery framework.
