Your new role:

Join the Geriatric Evaluation Management Rehabilitation Hospital in the Home (GEMRHITH) team at Eight Mile Plains Community Health Centre, where you'll deliver holistic, evidence-based nursing care that helps older adults recover and thrive in their own homes.  You will:

  • Provide advanced clinical assessment, planning, and coordination of care for patients with complex health needs in the community setting.
  • Work collaboratively within a multidisciplinary team to support hospital avoidance and post-acute rehabilitation services.
  • Deliver leadership and mentorship to nursing colleagues and students, supporting quality improvement and clinical education initiatives.
  • Contribute to evidence-based practice, research, and continuous improvement activities within Metro South services

About you:

You're a proactive and compassionate nursing professional who thrives on autonomy and teamwork. You bring advanced clinical judgement, a passion for community-based care, and the ability to inspire excellence in others. You:

  • Hold a current registration with AHPRA as a Registered Nurse and extensive clinical experience in community or acute care settings.
  • Have demonstrable advanced skills in geriatric or rehabilitation nursing, care coordination, and holistic assessment.
  • Have strong interpersonal and communication skills, with the ability to engage effectively with patients, carers, and multidisciplinary teams.
