**The Opportunity**
The City of Melton is located on the fringe of Melbourne in the rapidly expanding western corridor. The city is experiencing one of the fastest growth rates in the state and is currently seeking a creative and energetic person to fill one permanent part time Social Support Worker role across our Melton and Eastern sites as part of the Social Connections team within Community Care and Active Living.
The successful applicant will be responsible for assisting in the delivery of client-focused programs for both older people and people with a disability. Applicants must be self-motivated, innovative and responsive to the needs of participants and committed to working as part of a team. If you have the ability to engage and connect with the community and take pride in making a positive impact to people's lives, then this role is for you. The position reports directly to the Social Support Supervisor - Melton. This role will be responsible for contributing toward a strong positive culture within the Community Care and Active Living team and greater Melton City Council.
**Key Responsibilities**
- Assist and support clients to make choices with regards to their support requirements and community access using client directed care and active service model principles.
- Facilitate the day to day running of the Social Support programs including the development of individual and small group activities.
- Monitor the physical, emotional and social wellbeing of clients and report issues to the Social Support Supervisor.
- Fostering positive and respectful relationships, respecting cultural values and beliefs and individual choices.

**About you**
**_ To be successful in this role you will have the following:_**
- Certificate III in Community Services (Aged Care) and / or equivalent.
- Experience, understanding and a genuine commitment to working with frail older people, people with dementia and/or people with a disability and their carers.
- Experience in the preparation and delivery of social support groups in both centre and community based settings.
- The successful applicant must be available to work rostered hours including weekend and after hours work.
- First Aid Certificate Level 2.
- Current valid Victorian Driver’s Licence.
- Must hold or be able to obtain an Employee Working with Children Check.
