**The Opportunity**

The City of Melton is located on the fringe of Melbourne in the rapidly expanding western corridor. The city is experiencing one of the fastest growth rates in the state and is currently seeking creative and energetic people to fill two permanent part time Social Support Workers at our Melton site and one permanent part time and one casual position Social Support Worker for our Eastern site as part of the Social Connections team within Community Care.

The successful applicants will be responsible for assisting in the delivery of client-focused programs for both older people and people with a disability. Applicants must be self-motivated, innovative and responsive to the needs of participants and committed to working as part of a team.

**Key Responsibilities**
- Assist and support clients to make choices with regards to their support requirements and community access using client directed care and active service model principles.
- Facilitate the day to day running of the Social Support programs including the development of individual and small group activities.
- Monitor the physical, emotional and social wellbeing of clients and report issues to the Social Support Supervisor.
- Assist in planning, conducting and evaluating appropriate social support activities that are relevant to the individual and group needs for both centre and community based social support programs.
- Complete all administrative and system management tasks and assist with shopping to obtain resources, groceries and program requirements.
- Provide personal care (toileting), meals assistance and mobility assistance to clients as required.

**About you**

**_ To be successful in this role you will have the following:_**
- Certificate 3 in Community Services (Aged Care) and / or equivalent.
- First Aid Certificate Level 2.
- Experience in the preparation and delivery of social support groups in both centre and community-based settings.
- Communicate and work with frail aged people with a basic understanding of the ageing process, the effects of dementia and related conditions physical and sensory impairments, incontinence, psychiatric and intellectual disabilities.
- Current valid Victorian Driver’s Licence.
- Current valid Employee Working with Children Check.

The successful applicant must be available to work rostered hours including weekend and after-hours work.
All successful applicants will undergo a mandatory Police Check and Medical/Functional Capacity Test prior to commencing.
