**ABOUT US**

Documen Support Services is a Disability Service Provider based in the Liverpool region. We are currently expanding our team and seeking experienced support workers with the skills to hit the ground running. We are a tight knit team of dedicated support staff who are always willing to go above and beyond for participant satisfaction. Currrently we have need to service clients in various areas around Sydney.

**POSITION OVERVIEW**:
**Disability Support Worker (Casual Contractors needed)**

The Disability Support Worker is responsible for implementing training and support services aimed at maximizing the living, employment, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

**Qualifications**:

- Cert 3 Level or higher in relevant services desirable but not essential
- Current NDIS Worker Screening Check (Or prepared to attain)
- Completed NDIS Worker Orientation Module (Or prepared to attain)
- Current Police Clearance (Or prepared to attain)
- Current Drivers Licence
- Own transport (reliable vehicle with relevant safety standards)
- ABN - Current (or prepared to attain)
- Up to date Vaccination Certificates

**Duties and Responsibilities**:

- Assist with clients personal care needs
- Transport clients to appointments, health services, shopping or general community access
- Focus on achieving goals and objectives as per client's NDIS plan
- Maintain solid relationships with all stakeholders including other health professionals and providers
- Work with varying degrees of disability from levels 1 to 3
- Care for clients with disorders such as Autism, ADD, ADHD and other mental health issues
- Work with all age groups including both children and adults
- Provide individual support as well as participate in group activities
- Be flexible to work various shifts including mornings, afternoons and evenings
- Present accurate and professional client update reports as and when required
- Complete any incident reports and submit to manager within 24 hours
- Share your experiences on a weekly basis with fellow staff and management
- Be polite, couteous and professional at all times
- Punctuality is a key asset to maintain reliability and trust with clients
- Travel will be required to regions within and out of the Liverpool area

**Job Type**: Contract

**Salary**: $31.00 - $33.00 per hour
