**Care Navigator**

**The Opportunity**

We are seeking experienced Registered Nurses & Allied Health Professionals to work remotely who will be responsible for care coordination, navigation and health coaching services via face-to-face or telehealth or virtual to people that have a chronic condition and complex needs. This involves developing an individualised care plan, referring to other providers as needed, navigation across multi-disciplinary services required to fulfil this care plan, service linkage such as transport, and supporting self-management principles.

The shifts will be 08:00am - 4:00pm Monday to Friday (No Weekends or Public Holidays).

**About You**
- Relevant qualification in Nursing (Registered Nurse) or Allied Health with demonstrated experience in field of expertise
- Experience in delivering chronic condition management, health care navigation, health coaching and the principles of health literacy
- Well-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families, and other health care professionals
- Demonstrated ability to provide education, support and coaching to clients and their carers
- Demonstrated time management and organisational skills to effectively plan and prioritise work
- Demonstrated ability to work both autonomously and with a team to participate and contribute to program and organisational objectives
- A demonstrated empathetic and flexible approach in all interactions with clients, carers and other health providers
- Based on Silverchain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19 including booster, or evidence of a medical exemption in order to commence employment with Silver Chain.
