**The opportunity**

Working as part of our multidisciplinary healthcare team, you’ll provide phone-based health coaching for clients at risk of, or currently living with chronic health conditions across Australia. Your clinical environment will be driven by human-centered, holistic, evidence-based care and supported by generous professional development.

The team are currently working hybrid with flexible WFH opportunities implemented. You will be based at our Spring St, Melbourne CBD office.

**What you’ll be doing**
- Supporting clients with a focus on preventative health and helping clients navigate their management of chronic diseases and comorbidities
- Using telehealth to share behaviour change principles so clients can establish personal goals and implement strategies to understand and manage their health for the long-term
- Building partnerships with internal teams to exchange client referrals and promote the work we do
- Improving the health and wellbeing of hundreds of Australians.

**Your skills and experience**
- Qualification in dietetics with current DAA registration
- Excellent communication skills, both written and verbal, including rapport building on first contact
- Experience managing behaviour change in clients living with or at risk of developing chronic health conditions
- Enjoyment of phone-based work with a compassionate, empathetic and professional approach
- Strong administration and time management skills, tech savvy
- Passionate about health and committed to making a real difference in the community.
