As the Clinical Nurse/ Midwife the role will be responsible for coordinating access to community delivered services as required to support continuation of care, providing high-quality person-centred care and preventing unnecessary ED presentations. Staff in this role are required to maintain contemporary knowledge about safe clinical triage, hospital avoidance strategies and community alternatives for the delivery of care. The role will provide nursing services via Telehealth in the South Australian Virtual Care Service (SA VCS) utilising knowledge and skills that have been consolidated by experience and/or further study.

**About the role**:
Reporting to the Nursing Director Operations, Virtual Care Service as the Clinical Nurse/ Midwife duties for the role will include:

- Provide proficient, person centred, clinical nursing care and/or individual case management to patients/clients in a defined clinical area,
- Monitoring patient/client care plans to ensure appropriate care outcomes are achieved,
- Responsibility for their own practice standards, activities delegated to others and the guidance and development of less experienced staff,
- The SA Virtual Care Services is located at Tonsley, and staff are required to work onsite

**About you**:

- Effective communication, problem solving, conflict resolution and negotiation skills,
- Ability to prioritise workload and meet set timelines, whilst working under mínimal supervision,
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate,
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards

To be considered for the RN2C (Clinical Nurse) level you must have at least three years, full time equivalent, and post registration experience.
