Join BASScare as a Community Services Intake Officer and play a key role in helping older adults access the support they need to remain independent, connected, and well.

**About the Role**:
As the first point of contact for many new clients and families, you’ll guide people through their aged care journey. You’ll conduct assessments, develop tailored care plans, and coordinate services across a range of programs, ensuring each person receives the support that best meets their needs.

**Key Responsibilities**:

- Conduct client intakes and assessments (in-home, over the phone, or in-centre)
- Develop, review and update individual care plans in partnership with clients, carers, and health professionals
- Monitor changing client needs and ensure services remain responsive and appropriate
- Maintain accurate, high-quality records and support compliance and audit processes
- Build strong relationships with clients, internal teams, and community stakeholders
- Contribute to service improvements and quality assurance activities

**What You’ll Bring**:

- Experience with client intake, assessment, and care planning.
- Solid understanding of consumer-directed care principles
- Strong admin, IT and organisational skills
- Excellent communication, interpersonal, and problem-solving skills.
- Comfortable working independently and collaboratively
- Committed to inclusion, dignity, and person-centred practice
- Police Check (current) and valid Driver’s Licence.

**Desirable but not essential**:

- Familiar with government-funded aged care programs like My Aged Care, CHSP, or Support at Home.
