**About us**

Care Nation Disability services is a disability support organisation based in South East Brisbane looking for quality support workers who value continuity of support. We have a person centred approach and our focus is to match our participants with support workers who they will be able to form a great relationship with to achieve their goals.Successful applicants will require flexible availability including morning, afternoons, nights, sleepovers and weekends.

We value our support workers and understand the importance of a good work/life balance therefore we work closely with our staff to ensure they are happy within the workplace.

**Qualifications & experience**
- Certificate III in Individual Support or equivalent required
- NDIS worker orientation module
- Experience with any of the following will be highly regarded - ASD, Down Syndrome, Mental health, Personal care.
- Current first aid & CPR
- Current Blue Card
- Current Yellow Card or NDIS Worker Screening Check
- Current national police check (or willing to obtain)
- Open license
- Full comprehensive vehicle insurance including roadside assist
- Access to a smart phone

**Tasks & responsibilities**
- Individualised community-based support
- Support & skill building in home and in the community
- Working with participants to develop independence and daily living skills
- Providing personal care to participants
- Contribute to the ongoing development of a person-centred team
- Assist with medications and other duties as specified as per the participant support plan
- Maintain participant records to ensure consistent and quality care
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning and reviews
- Be adaptable to changing circumstances and being able to priorities work
- Display a client focused and person-centred manner
