**About the Role**

This is a meaningful leadership opportunity within Grampians Health’s Mental Health and Wellbeing Services, supporting individuals at significant risk of suicide. The HOPE Program (Hospital Outreach Post-suicidal Engagement) provides clinical and psychosocial outreach for individuals following a suicide attempt or experiencing serious suicidal ideation.

As Team Leader, you’ll lead a multidisciplinary team that works with clients and their support networks to strengthen protective factors and reduce risk. You’ll play a vital role in ensuring service quality, accessibility, and alignment with strategic reform across the Grampians region.

**Key Responsibilities**
- Lead and oversee daily operations of the HOPE team
- Coordinate outreach services and client care pathways
- Supervise and support clinical team members
- Promote service quality, safety and client-centred care
- Contribute to service development and reform initiatives
- Liaise with internal stakeholders and community supports

**Skills & Experience**
- AHPRA registration (RN Div 1, OT, Psych) or qualified Social Worker
- Mental health postgraduate qualifications highly regarded
- Strong clinical knowledge and suicide risk management
- Proven supervision and leadership capability
- Experience in community mental health or related services
