**Hospital Outreach Post-Suicidal Engagement (HOPE) Clinician**

Up to $20,000 incentive payment may be available to support relocation

The Victorian Government has allocated funds to support rural and regional Area Mental Health Services (AMHS) to provide incentives to attract workers to roles that are difficult to fill.
The new workforce relocation and incentive grants program is being administered by Rural Agency Workforce Victoria (RWAV).

**About the Role**

The primary aim of the Grampians HOPE program is to reduce rates of suicide in the region through assertive outreach support to individuals that have been identified at risk. Participants and their support networks, are offered tailored interventions for a period of up to 12 weeks, with the primary aim to build protective factors against suicide, address issues that may reduce future risk of suicide and to reduce associated distress.

The Psychosocial Support Worker will work collaboratively with the multidisciplinary team, which includes peer workers and clinical staff, assisting participants to navigate services, offer linkage and referral, provide practical support when required, offer support to carers and family and develop and assist with safety planning.

The Grampians HOPE program will be expanding referral pathways and extending hours of service in the future

**KEY ACCOUNTABILITIES**

Working under the direction of the Clinical Lead:
Provide comprehensive mental health assessment and treatment plans to people referred to HOPE program for secondary consultation or intake into the 12-week psychosocial program.

Develop expert knowledge in the field of Suicide Prevention and evidence-based approaches to reduce suicide rates in the Grampians region.

Provide evidence-based interventions for consumers post suicide attempt, engaging in significant deliberate self-harm, or voicing suicidal ideation.

Share skills and knowledge with key groups, formally and informally, to strengthen and enhance existing programs and clinical responses to people who may present with risk factors for suicide.

Effectively and efficiently utilise available resources to ensure the delivery of a supportive, safe, recovery-oriented & family/carer inclusive experience for all consumers of the HOPE Program.

Provide support, information, and education to family members, carers and significant others.

Actively and effectively work within a multidisciplinary team and contribute to the generic and discipline specific expertise of the team, providing discipline specific education and supervision to students on placement where appropriate.

Maintain thorough, legible and accurate documentation in medical records. Complete statistics and outcome measures, as per HOPE service delivery guide.

Actively participate in local, statewide and national forums relevant to the continuous development of the Grampians Area HOPE Program and expansion of suicide prevention and postvention strategies within the community.

Compliance with all Policies and Procedures including infection control policies, risk and quality systems and legislative requirements.

Participate in organisation wide integrated risk management and quality improvement systems by being aware of responsibilities to identify, minimise and manage risks identifying opportunities for continuous improvement in your workplace through communication and consultation with managers and colleagues.

May be required to work extended hours, including weekends and Public Holidays to meet operational needs as the service expands.
