About the Role
The Case Manager are responsible for managing case plans with all clients within the Settlement Services Division - SRSS Program. This includes conducting comprehensive assessments and developing and reviewing case plans
- Conduct comprehensive assessment of client needs
- Provide high quality case management support to clients
- Adherence to contractual requirements of the funded program & meeting of any deadlines
- Develop comprehensive and achievable care plans outlining services required by the client and related costs
- Identify, discuss and monitor complex client issues and undertake risk assessments as required
- Provide appropriate information and referral support and develop and maintain professional links with other relevant services and agencies
- Provide support, guidance and supervision to the bi-lingual bicultural staff and volunteers
- Complete monthly case updates and other reports as required
- Keep accurate and up to date client files and case notes
- Ensure case plans are reflective of client needs and support with monitoring of client progress
- Ensure compliance with the program’s Operational Manual
- Participate in organisational programs and events
- Other duties as required to meet operational needs
Skills & Experience
- A relevant tertiary qualification in social work, social or behavioural sciences and/or demonstrated experience in the provision of casework services in the health or welfare field.
- Advanced casework/case management skills
- Excellent communication (verbal and written) and interpersonal skills
- Well developed negotiation, advocacy, conflict resolution and decision making skills
- Demonstrated ability to work with people in stressful situations
- High level Problem solving abilities and the capacity to work autonomously
- Proven networking, liaison and casework advocacy skills
- Ability to manage complex and sensitive issues with appropriate discretion in a highly political environment
- Demonstrated experience in the provision of culturally appropriate and sensitive services to people of CALD background is DESIRABLE
