We specialise in providing NDIS, Aged Care, and State-based community and residential services. Our person-centred philosophy, "Everything about you, with you" ensures we work collaboratively with our clients to meet their needs. **Role Details** The Support Coordinator is a role within DHS Disability Services and is accountable to the Senior Support Coordinator for: - supporting clients and NDIS participants through a centralised and coordinated intake process to access services in accordance with individual goals outlined in their NDIS plans, providing a service that aligns with the NDIS practice standards and person-centred care model - working collaboratively with people living with disability, their families and carers to improve supports of planning and aims to improve quality of life of individuals - coaching and supporting clients to build capacity and resilience within their network through NDIS funded supports, mainstream services, community and informal support services - providing advice and facilitating education to clients, their families and carers about building capacity through the NDIS and supports available - managing complex queries and issues and providing support on a case-by-case basis - maintaining competency in all compliance requirements related to responsibilities. **Special Conditions** National Police Check required Employment-related Screening Check required Some interstate and intrastate travel may be required Must hold a current Australian issued driver’s licence (equivalent to minimum class C). **Role Status**:Term up to 30/06/2026. **Salary Range**:$76,561-$80,256 **Location**:Woodville **Diversity and Inclusion** DHS is committed to making our services and workplaces safe and inclusive for all people and ensuring the full diversity of the communities we serve are represented in our workforce, including Aboriginal and Torres Strait Islander people, people with disability and/or neurodivergence, young people, older people, culturally and linguistically diverse people, LGBTIQA+ and gender diverse people.
