**Position Summary**

Are you passionate about providing effective and high-quality care management services to clients in need? We are seeking a dedicated Clinical Care Team Leader to join our team at Home Care and Disability Services. As a Team leader, you will be coordinating Arcare casual RNs for in-home clinical services. The role also involves mentoring staff, managing client care plans, and supporting complex client cases while fostering best practices and continuous improvement.

**Key Responsibilities**
- Conduct clinical assessments and providing specialised care.
- Leading, mentoring, and supporting the Care Consultants (CC) team to ensure high quality home services.
- Liaise with healthcare professionals, community agencies, and other stakeholders to coordinate care.
- Oversee the allocation and case management of clients.
- Support the CC team with complex or high risk cases.
- Manages client budgets, networks with external health and community services
- Maintain accurate client records and comply with legislative requirements and organizational policies.
- Contributes to business development initiatives.
- Participates in strategic planning, quality improvement, and compliance audits to ensure the organization’s clinical care services are of the highest standard.

**Qualifications and Skills**
- Tertiary qualification in Nursing, Allied Health, Social Work/Welfare, or related discipline.
- Current required vaccinations and Right to Work in Australia.
- Experience in client assessment and care management, preferably in aged care.
- Excellent communication, problem-solving, and decision-making skills.
- Ability to work effectively in a team and autonomously.
- Knowledge of funding available in the home care space and relevant legislation.
- Computer literacy, including Microsoft Office and database systems.

**Desirable**
- Experience using Carelink.
- Current State Driver’s License.
