Sydney, Australia Gemme Support Services & Therapies Full time

We are looking for **Support Workers** to join an exciting team at Gemme Support Services and Therapies based in Sydney, that include but not limited to:

- Daily life tasks for the NDIS client
- Daily personal activities for the NDIS client
- Assist the NDIS client with travel/transport
- Assist the NDIS client with household tasks
- Assistance with daily personal activities needs for the clients
- Assistance with daily life tasks in a group/shared living setting (Supported Independent Living - SIL )
- Assistance with client engagement with community and social activities
- Assist clients to travel to and from activities/appointments as required
- Development of daily living and life skills of the client
- Essential household tasks
- Assist the client in the participation of community, social and civic activities
- Assist the client in group/centre based activities
- Assisting with the development of, and ongoing planning and assessment of client goals
- Maintaining daily notes for client activities (must have access to a note taking system on a laptop/computer)
- Completion of required reports and documentation
- Ongoing and positive communications with other client stakeholders, such as carers, family and staff

**Essential criteria**

**Must haves**:

- ABN and own insurance
- Working with Children Check
- Current First Aid and CPR qualifications
- Police check (or willingness to obtain)
- Completed COVID-19 vaccinations
- NDIS modules completed (or willingness to obtain)
- Current NDIS Worker Screening Check or equivalent (or willingness to obtain)
- Minimum Certificate III in Individual Support (Disability) or a completed/ongoing degree in Medicine, Nursing or an Allied Health field
- Minimum 12 months experience as a Disability Support Worker or equivalent

**Other Requirements**:

- Willingness to travel.
- Driver's licence and a safe, reliable vehicle with insurance
- Home office space or the ability to create an appropriate working space in your home (if appropriate)
- An understanding of the NDIS, NDIS plans and related legislation.
- Knowledge of the NDIS Quality and Safeguards code of conduct and service standards.
- Solid understanding of the NDIS support coordination role and responsibilities.The role offers excellent learning and development opportunities, providing support to participants in the NDIS. You will have the opportunity to work from home (if appropriate) and in the home of NDIS participants.

**Job Type**: Contract

Pay: $30.00 - $45.00 per hour

Schedule:

- Monday to Friday
- Weekend availability

Work Location: Hybrid remote in Sydney, NSW 2000
