Austin Health is the major provider of tertiary health services, health professional education and research across 3 campuses in the northeast of Melbourne and state-wide. An exciting opportunity currently exists for an enthusiastic and experienced Case Manager to join our Transition Care (TCP) and Restorative Care (RC) Programs which form part of the Aged Care Community Services, located at the Heidelberg Repatriation Campus.

TCP is a goal-oriented, time limited program which supports older people at the conclusion of a hospital episode, who require more time and support in a non-hospital environment to complete their restorative process, optimise their functional capacity and finalise and access their long-term care arrangements. The RC program is modelled on the TCP model of care.

The TCP Case Manager completes detailed individual assessment and care planning for eligible older clients of the Austin Health Transition Care or Restorative Care Programs focusing on the client’s abilities, lifestyles and interests. The Case Manager then provides a period of case management, intensive service provision and / or therapy within their own home or aged care facility, enabling discharge to other community supports or to an aged care facility at optimal level of functioning

To be successful in this role you must demonstrate:

- Current AHPRA registration as Registered Nurse with current or Allied Health Professional
- Extensive experience in aged care
- Advanced assessment and care planning skills
- Demonstrated understanding of the factors that impact on older people with significant care needs
- Understanding of the range of services available in the community, and how to package them appropriately to meet client needs.
- Ability to communicate effectively in both written and verbal forms
- Ability to problem solve, prioritise
- Demonstrated negotiation skills
- Ability to work independently and within a multi-disciplinary team
- Commitment to quality, best practice and environmental safety
- Hold a current Victorian Drivers Licence
