**Position**:Community Worker
**Office Location**: Strathpine
**Community Support Responsibilities**:
- Identify needs of individuals with disabilities to create and implement effective support programs
- Conduct interviews to understand personal challenges and determine optimal NDIS support pathways
- Track clients' progress and provide ongoing support to help them achieve positive outcomes
- Evaluate and connect clients with resources in health, housing, education, and employment
- Develop working relationships with disability services, mental health services, and the National Disability Insurance Agency
**Client-Centric Approach**:
- Use a strengths-based approach when developing support plans
- Help clients increase community access and build capacity for greater independence
- Assist clients in coordinating supports and managing crisis points
- Raise awareness about available social services and programs
**Stakeholder Collaboration**:
- Maintain effective working relationships with various service providers and agencies
- Maintain accurate records and documentation
- Support clients through changes in circumstances, including NDIA plan reviews
**Essential Criteria**:
- Previous, relevant experience in a similar role
- Qualifications in human services or related field
- Excellent communication and active listening skills
- Strong problem-solving and decision-making abilities
- Experience in case management, advocacy, or program coordination
- Compassionate and culturally aware approach
