We are seeking a motivated and experienced Client Integration Manager to join our team. As a Client Integration Manager at Special Care Australia, you will play a crucial role in ensuring the seamless integration of our clients into our programs and services. You will work closely with clients, their families, and our internal teams to develop personalized integration plans and coordinate support services to meet their individual needs.

**Key Responsibilities**:

- Develop and implement individualized integration plans for clients, ensuring their participation in our programs and services is seamless and tailored to their needs.
- Collaborate with internal teams and external stakeholders to coordinate support services and resources for clients.
- Conduct assessments to identify client needs and preferences, and make recommendations for program adjustments or enhancements.
- Provide guidance and support to clients and their families throughout the integration process, addressing any concerns or challenges that may arise.
- Monitor client progress and outcomes, and make adjustments to integration plans as needed to ensure positive outcomes.

**Requirements**:

- Bachelor's degree in social work, psychology, disability studies, or a related field.
- Minimum of 3 years of experience working in a similar role, preferably in the disability support sector.
- Strong interpersonal and communication skills, with the ability to build rapport and trust with clients and their families.
- Knowledge of the NDIS framework and relevant legislation governing disability support services in Australia.
- Ability to work collaboratively in a multidisciplinary team environment.

Pay: $90,000.00 - $120,000.00 per year
