**ABOUT THE ROLE**
We are looking for exceptional Care Coordinators who are passionate health/aged care professionals looking to join our Caloundra and Caboolture offices on a full-time basis. As Care Coordinator you will be an integral point of contact for workers and clients to provide support for the broader community team.

As a **Care Coordinator** you will be responsible for:

- Building strong and meaningful connections with clients and their family
- Conducting comprehensive client assessments and development of care plans to support our clients to remain within their home
- Interacting and providing professional health and clinical care advice and direction to a team of home care workers
- Professionally liaising with clients and their families on a regular basis, addressing any concerns and service satisfaction
- Ensuring all practices are consistent within policy and legislative frameworks
- Maintaining up to date client records and information

To be successful you must hold:

- Formal tertiary qualification eg. Bachelor of Nursing or equivalent
- Current AHPRA registration as a Registered Nurse
- Demonstrated knowledge within the Aged Care sector
- National criminal history/police check
- Industry applicable licences (eg. AHPRA registration, BC/NDIS)
- Valid First Aid and CPR certificate
- Minimum of 2 COVID vaccinations
- Open drivers licence and reliable transportation
