About the role

We’re looking for an experienced and passionate Clinical Care Coordinator to join our team. This pivotal role provides clinical governance, oversight, and operational management across both NDIS and Home Care Packages.

You’ll be responsible for overseeing complex client care plans, ensuring our Community Support Professionals are trained and assessed in advanced clinical skills, and upholding the highest standards of care, compliance, and professionalism.

Your leadership will ensure smooth service delivery, effective risk management, and a supportive, well-trained team committed to delivering excellence to our clients.

Responsibilities

- Provide clinical leadership and continuous improvement to ensure quality, compliant services.
- Oversee complex care plans, medication administration, and clinical interventions (including wound care, catheter care, stoma care, palliative care, and more).
- Deliver training, coaching, and skills assessment for Home Care Workers.
- Manage incident and risk processes and ensure timely resolution.
- Assess and support customers living with dementia or cognitive impairment.
- Conduct falls risk assessments and implement prevention strategies.
- Collaborate with healthcare professionals, clients, and families to ensure best-practice outcomes.

What you’ll bring

- Current AHPRA registration as a Registered Nurse.
- Evidence of continued professional development
- Minimum 3 years’ experience in aged care or community care.
- Proven leadership skills with the ability to mentor and develop staff.
- Strong knowledge of Aged Care Quality Standards and relevant legislation.
- Experience in complex clinical assessments and care planning.
- Proficiency in Microsoft Office (Word, Excel, Outlook).
- Current National Police Check, Driver’s Licence, First Aid & CPR Certificate
