**About the roles**
We have exciting opportunities to provide newly Certificate III or IV with experience in community aged care and disability care industry within a supportive environment.
Working along side a more experienced Registered Nurse, you will be a Care Team Leader providing quality front line care to our consumers as well as overseeing a dedicated team of carers.
These roles are designed to give you a broad range of experiences as well as informal and formal professional development opportunities to develop your Support coordination skills to prepare you for a long and successful career in Community Care.
**About you**:
- Certificate III or IV in Disability Care or Community Care
- Demonstrated experience in coordinating Disability, community, health or social services.
- Sound knowledge of NDIS legislation, policies and procedures
- Excellent planning, organisational and management skills
- Problem solving skills and strong attention to detail
- An understanding of the principles of service coordination and case management, including client centred assessment, planning, provision and evaluation of services
- Basic computer skills including Microsoft Office.
- Current driver's license and own a car.
**Your key responsibilities will include**:
- Ensuring clients wishing to be looked after in their own home are provided with services that are compliant with Aged, Disability Care Legislation and standards
- Deliver best practice flexible care for clients in line with the Home Care Standards
- Build strong relationships with clients and their families
- Develop strong professional networks with local stakeholders
- Undertake client assessments, care planning and case management in consultation with the client.
- Lead, manage and support all staff in the delivery of person centred services
- Participate in Quality Improvement activities
- Achieve budgeted goals whilst leading our people to achieve outstanding results for our clients
- You will be part of a team of long established Home Care Coordinators currently providing services across Melbourne
