**What You Will Be Doing**

The Early Intervention in Chronic Disease (EIiCD), carer support and social support group program plays an important role in supporting frail older people with chronic and complex conditions, younger people with a disability, and their carers. Integrated chronic disease management refers to the provision of person centred care by services working together with the person to ensure coordination, consistency and continuity of care over time and through different stages of their condition. The Program Manager will work within the EIiCD leadership team to facilitate and provide direction for ongoing quality improvement in service development and delivery within the EIiCD, carer support and social support group program.

**What You Need**

We anticipate you will have:

- Relevant tertiary qualifications in Allied health profession or Nursing.
- Full AHPRA registration with no restrictions to the scope of practice.
- Previous Program Leadership and management experience.
- Extensive experience as an Allied Health Professional or Registered Nurse.
- Experience in preparing and monitoring expenditure against budgets.
- A current and valid Victorian Drivers License.

**Desirable Criteria**
- An understanding of the concepts and strategies of population health promotion and the chronic care model
- Post Graduate qualification in; research, business, education, management, health administration or public health Experience of working within a community health environment**What Next**

**Additional Requirements**
- Please refer to salary circular for Allied Health Professional (VICTORIAN PUBLIC SECTOR) (SINGLE INTEREST EMPLOYERS) Enterprise Agreement 2021-2026 OR Nurse and Midwives (VICTORIAN PUBLIC SECTOR) (SINGLE INTEREST EMPLOYERS) Enterprise Agreement 2020-2024 for salary range. Sub-grading will be based on years of experience.
