**What You Will Be Doing**

The Community Care Social Worker is responsible for coordinating intensive, short term (3-6 months) outreach care, and delivering allied health interventions within their scope of practice to clients who have chronic health conditions and / or complex health needs.

The role involves facilitating collaborative integrated care planning across acute and community-based services. Care coordinators support people to navigate the service system and services. They provide the point of contact as people undertake their care plan.

**What You Need**

We anticipate you will have:

- Social worker with current registration with Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organisation, or eligible for AASW membership.
- Demonstrated ability to manage multiple competing tasks and effectively prioritise.
- Demonstrated experience working with people who are at-risk and vulnerable and have complex needs.
- Specialist experience and knowledge regarding chronic disease management.

Essential: Demonstrated experience in completing comprehensive Social Work assessment and intervention.

Essential: Demonstrated experience working with at-risk and vulnerable people with complex and chronic health needs.

Essential: Demonstrated ability to manage multiple competing tasks and effectively prioritise.

Essential: Demonstrated experience in working independently and as part of a multidisciplinary team, with the ability to work effectively, supportively and flexibly.

Essential: Current Victorian Drivers Licence.
