Job Overview

The Home Health Care Specialist plays a vital role in delivering high-quality care to individuals with chronic health conditions and complex healthcare needs. Our team is committed to providing responsive, patient-centric outcomes for all.

Key Responsibilities
  • Provide comprehensive OT assessments in home settings and provision equipment needs
  • Coordinate intensive short-term outreach care and deliver allied health interventions
  • Facilitate collaborative integrated care planning across acute and community-based services
  • Support at-risk and vulnerable people with complex needs
Requirements
  • Occupational Therapist registration with Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated experience in providing comprehensive OT assessments in home settings
  • Specialist experience and knowledge regarding chronic disease management
  • Ability to work independently and as part of a multi-disciplinary team
