We are seeking a highly motivated and compassionate Case Manager to join our team at Premier Disability Care Services. As a Case Manager, you will be responsible for overseeing and coordinating the delivery of support services to our clients, ensuring their individual needs are met and their goals are achieved. You will play a pivotal role in advocating for the rights and well-being of individuals with disabilities.
**Responsibilities**:
- Conduct comprehensive assessments of clients' needs, goals, and support requirements.
- Develop individualized care plans in collaboration with clients, their families, and other stakeholders.
- Coordinate and monitor the delivery of support services to clients, ensuring quality and adherence to NDIS guidelines.
- Provide ongoing support and guidance to clients and their families, addressing any concerns or issues that may arise.
- Advocate for clients' rights, ensuring they have access to appropriate services and resources.
- Liaise and collaborate with other service providers, healthcare professionals, and community organizations to ensure coordinated care.
- Maintain accurate and up-to-date records of client interactions, progress, and service delivery.
- Stay updated on NDIS policies, procedures, and guidelines to ensure compliance and provide accurate information to clients and staff.
**Qualifications and Skills**:
- Bachelor's degree in Social Work, Psychology, Disability Studies, or a related field. A social work background is beneficial.
- Previous experience in a case management role, preferably within the disability sector.
- Knowledge of the NDIS and its principles, guidelines, and funding models.
- Strong assessment, planning, and coordination skills.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients, their families, and external stakeholders.
- Ability to work independently and as part of a team, with strong organizational and time management abilities.
- Empathy, patience, and a genuine commitment to improving the lives of individuals with disabilities.
- Current driver's license and access to a reliable vehicle.
