**Position Purpose**

The **Disability Services Officer** (DSO) is responsible for delivering high-quality, person-centred support to clients with **acquired brain injuries** and **High Complex Needs**. This role focuses on the assessment of individual needs, development and implementation of tailored support and training programs, monitoring of progress, and coordination of referrals to appropriate community and health services.

DSOs promote community inclusion, skill development, and independence while liaising with internal and external stakeholders to ensure a holistic and strengths-based approach to client support. The role will vary depending on individual needs, program areas, and funding requirements.

All staff are expected to operate within the principles of person-centred planning and community integration, in line with arbias’ commitment to promoting independence, choice, and control for all individuals supported.

**Client Assessment & Planning**
- Interview clients and assess the nature and extent of their difficulties, strengths, and goals.
- Undertake comprehensive needs assessments focused on health, wellbeing, housing, social connection, and vocational aspirations.
- Plan, develop, and implement educational, training, and support programs tailored to individual needs, using a goal-oriented and evidence-based approach.

**Care Coordination & Referral**
- Coordinate and facilitate access to internal and external services that enhance client wellbeing, including mental health, housing, employment, healthcare, and community participation.
- Refer clients to appropriate support agencies for additional help and act as the liaison to ensure a coordinated approach.
- Advocate for client access to relevant services, funding, or entitlements.

**Monitoring & Evaluation**
- Monitor and report on the progress of clients using strength-based tools and regular review processes.
- Adapt support plans in collaboration with clients, support coordinators, and relevant stakeholders to meet changing needs.
- Maintain timely and accurate documentation, including case notes, incident reports, and outcome tracking.

**Community Engagement & Resource Development**
- Assess community needs and resources that relate to health, housing, education, employment, and other relevant services.
- Liaise with community groups, government bodies, welfare agencies, and private businesses to raise awareness of services and promote inclusion of people with disabilities and ABI.
- Build collaborative networks and partnerships to support integrated and holistic service delivery.

**Organisational Contribution**
- Use the Client File Management System to record case notes, communications, and documentation in accordance with arbias policies and shift requirements.
- Attend and actively contribute to client meetings, planning sessions, care team reviews, and supervision.
- Support clients during appointments and community activities that align with their goals.
- Comply with all arbias policies, including those related to incident/hazard reporting, infection control, and occupational health and safety.
- Participate in ongoing professional development, training, team meetings, and service planning activities.
- Be adaptable to changing service needs and duties as requested by the House Supervisor, Team Manager, Client Services Manager, or CEO.

**Disability Support in Residential Services**
- Provide direct support to clients residing in arbias-managed residential services, in line with individual support plans.
- Promote skill development and independence in daily living, social participation, and self-management within the home environment.
- Collaborate with other team members and stakeholders to maintain a safe, supportive, and inclusive residential setting that reflects the principles of person-centred active support.
- Contribute to house routines, structured activities, and support strategies that align with client goals and therapeutic recommendations.

Use of fleet or personal vehicles to be recorded as the company policy. This includes, vehicle log sheets, reporting damage to a vehicle and comply with the relevant Road Safety Act applicable to your state.
