CarePro Pty Ltd - We are registered NDIS Provider based in Melbourne's South East. We are provide range of NDIS services including Accomodation, Core Supports and Support Coordination Level1 and Level2.

**The Role**:
We are looking for an experienced NDIS Support Coordinator, who has at least 12 months experience working as a support coordinator. You will be working remotely most of the time and occasionally in the office for meetings.

The role will be casual, starting with one full day or preferable 2 half days but the potential to be 2-3 full days in the near future.

**You will be required to**:

- Manage caseload management
- Plan Implementation
- Help participants understand funding flexibility and limitations
- Connect participants to preferred services
- Continue to Progress and Monitoring funds and services
- Work within the funding levels for each participant
- Submit relevant reports to the NDIA in a timely manner.
- Maintain progress notes and records of hours of support provided
- Develop and maintain effective networks with the other disability and mental health services
- Build effective relationships with clients and their families while maintaining appropriate and professional boundaries
- Help to resolve points of conflict if problems arise
- Provide written reports, as required, on outcomes, successes, and barriers to support participant funding reviews.
- Gather and submit reports from relevant funded service providers to support the participant's funding review.

**You must have**:

- Must have Experience working as a Support Coordinator
- NDIS Worker Screening Check
- NDIS Workers Orientation Certificate
- Current and valid Police check
- Current and valid Working with Children Check
- Current and valid working visa in Australia
- First Aid and CPR
- Vaccinations against Covid including booster
- Valid Victorian Drivers License
- Comprehensive Car Insurance
- Certificate IV in Mental Health or Mental Health peer work or similar training and/or 2 years paid experience in supporting people with Mental Health challenges.
- Knowledge of the _NDIS Act 2013_, _Disability Act 2006 _and other relevant legislation.
- Excellent communication skills, both written and verbal
- Competent in the use of the latest computer software programs and is able to enter, modify and extract data accurately, including the ability to track funding package expenditure.

**Job Types**: Part-time, Casual
Part-time hours: 10 per week

**Salary**: $40.00 - $58.52 per hour
