Seeking NDIS Case Manager’s you will assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan.

You will also be required to establish strong rapport and working relationships with a range of stakeholders including NDIS providers, Adults and children living with Disability & their family members, NDIS Local Area Coordinators, NDIA Planners, internal and external service providers.

Key responsibilities are:

- Assessment, reporting, and interventions for clients identified as complex needs.
- Undertake informed referrals to appropriate allied health professionals and/or service providers.
- Initiate and maintain productive relationships with clients, stakeholders, informal supports, and service providers who may present with differing priorities.
- Manage client's NDIS plan to ensure services are being delivered.
- Develop and provide reports on outcomes and success indicators, within the required timeframes.
- Develop strategies which will address the identified needs of the participants in consultation with their representative/s.
- Support clients and their stakeholders, to understand the supports in their plan, navigating NDIS processes, identifying, and accessing mainstream, community and funded supports.
- Exceptional customer service, negotiation and influencing skills.
- Excellent ability to connect and build rapport with multiple stakeholders.
- Strong interpersonal skills and high levels of resilience.
- Excellent verbal and written communication skills.
- Degree in Social Work, Psychology, Counselling

Social Workers

**Job Type**: Subcontract

Pay: Up to $50.00 per hour

Expected hours: 5 - 20 per week
