**Disability Support Worker Positions**

**Introduction**:
Are you enthusiastic about creating a positive difference in the lives of individuals with disabilities and their families?

**We are currently looking for support workers available and willing to work with a our participants living in Croydon, Moroolbark, Lilydale and sorrounding areas. Support workers must have experience in Mental health including behaviours of concern. Worker will be required to provide supports in clients homes, and take participants out into the community. They must have a full driver’s licence and Comprehensively insured vehicle.**

**Your Responsibilities**
- Foster a supportive relationship with participants and their families
- Practice a person-centred approach that upholds human rights
- Comply with policies, procedures, and code of Conduct of Trinity Nursing Services
- Provide support with personal care tasks, including activities of daily living, domestic support, meal preparation, transportation, and engagement in social and community activities.
- Complete client case notes and other documentation promptly and accurately.
- Maintain Strict confidentiality both internally and externally
- Embrace workplace diversity and foster an inclusive environment
- Welcome constructive feedback

**Essential Requirements**
- Cert IV in Disabilty or equivalent
- NDIS Worker Screening Check
- Comprehensive Car Insurance.
- CPR Certificate.
- First Aid Certificate
- Full Driver’s License.
- NDIS Worker Orientation Module Certificate
- Working with children check.
- COVID Vaccine Certificate
