**About the Role**

Resilience to Recovery is looking to hire a Support Coordinator to provide support for a wide range of NDIS clients with unique needs and requirements. In joining the Resilience To Recovery team you will embark on a rewarding journey that involves supporting families and individuals in implementing their NDIS plans and goals.

As an experienced Support Coordinator you will be responsible for connecting people to a suitable NDIS plan and helping to organise access to relevant supports. You will provide a variable level of coordination and transitions for people and individuals of up to 65 years old, with evolving degrees of complex needs and circumstances. You will provide ongoing support and coordination in helping participants connect with community based or mainstream programs with the aim of helping them achieve their individual needs.

We are looking for someone with exceptional knowledge of NDIS regulations and reporting, excellent service skills, good time management and to be able to work under pressure independently and within a team. Most importantly you are someone who has a deeply caring nature that is applied to every aspect of the job, especially in relation to the individuals and their families. Your ability to develop strong relationships with stakeholders, health care providers, the individual and their families is essential.

Important requirements:

- A degree or qualification in Social Work, Human Services, or related disciplines is necessary.
- Previous experience in preparing in-depth reports as per NDIS requirements is essential;
- Minimum two years’ previous experience in providing support coordination for individuals with complex support and service requirements.
- Prior experience working with culturally and Linguistically Diverse individuals.
- Demonstrated understanding of the NDIS and experience working within NDIS sector for a minimum of two years.
- Ability to exceed KPI's and multitask;
- Previous Support Coordination or Case Management experience is a must.
- Understanding and speaking Arabic is a bonus.

**Key Responsibilities**:

- Conduct initial assessments.
- Coordinate with service providers and supports, and effectively meeting client objectives and requirements.
- Liaise with government agencies, community partners and allied health providers.
- Obtain quotes for services, organise service agreements and monitor Participant plans.
- Provide frequent Support Coordination reports to the NDIA.
- Revise and prepare incidents and SIL reports for stakeholder meetings
- Finalise and submit detailed SIL documentation to the NDIA in preparation for assessments.
- Organize the collection of, and provide, detailed information regarding the participant’s support needs, capacity and goals to the NDIA both for SIL and non SIL participants.
- Monitor plan outcomes and expenditure to ensure the Participant’s services remain within the budget, document progress in reports.
- Independently handle a range of participant cases and ensuring quality of care.
- Locating and referring participants to allied health services with a focus on Recovery.

**Job Type**: Subcontract

Pay: From $60.00 per hour
