**Our client**

Our client is a growing business that operates all across NSW that provides support workers and home carers to people living with disabilities and older Australians who wants to live in their home to keep independence with assessed assistance.

**The Role - Full Time Care Coordinator**

Our client is seeking an exceptional Full Time Care Coordinator with a strong commitment to client care and a passion for helping vulnerable people within the community. This is a remote position but there is the expectation that there may be occasional travel within 50km north of Sydney CBD.

The Care Coordinator must be able to focus on the total health care needs of a participant and in doing so, work collaboratively with health care professionals and providers to facilitate those needs. It is important that the incumbent in this role has easy access to a range of healthcare services and providers.

Key responsibilities include:

- Conduct client care assessments and planning.
- Supervise and manage Support Workers.
- Conduct Intake processing, clinical care planning and reporting.
- Schedule appointments and in-home services to appropriate staff based on discipline, availability, and geographical requirements.
- Coordinate the provision of quality person centred care according to the clients need and services and care plans
- Provide in-home care services as needed (Newcastle location).
- After hours and weekend on-call
- Recruit and train staff
- Staying in touch with the participant to ensure their needs are met
- Conduct general administration tasks

**Who are we looking for?**

You are an experienced Care Coordinator with:

- a conscientious work ethic with a passion for helping people
- demonstrable experience conducting client care assessments and care planning
- strong communication (both written and verbal) skills with the ability to communicate with providers, participants and their families in clear and simple terms; and transition care plans effectively.
- the ability to confidently jump in on the floor and manage emergencies as they arise.
- exceptional organisation and time management skills.
- high computer literacy with the knowledge of Microsoft Office Suite.
- Understanding and ability to abide by privacy and confidentiality standards and regulations.

**Salary**: From $65,000.00 per year
