Complete Care Team has a holistic and wellness model of attendant care for people with high support needs, providing a quality home based person-centred service.

We are seeking a Disability Service Coordinator to join our friendly team in Blaxland. The role involves liaising with case managers, medical teams, families and the clients themselves.

We specialise in providing support for acquired brain injured clients and are looking for a service coordinator with industry relevant qualifications or demonstrable experience. Understanding of and experience in brain injury, spinal, bowel and catheter care, quadriplegic and disability support is preferred.

**Key Responsibilities**:

- Day to day management of client’s needs
- Regular evaluation of client’s services
- Drafting of client Individual Support Plans
- Assistance to relieve on rotating roster of after-hours phone service
- Liaising with Case Managers and other professionals involved in a client’s care
- Compliance with WH&S legislation and company policy and procedure

**Essential Criteria**:

- Understanding of and transferable skills for the assessment, support and evaluation of clients, using evidence-based practice, relevant to home based care
- Commitment to the delivery of person-centred services, quality service for positive client experience; and behaviour consistent with CCT values.
- Demonstrated Leadership skills and attitudes: Thinks and works strategically o Achieves results and drives accountability o Engages people to build positive relationships o Exemplifies personal drive and integrity o Communicates and collaborates with influence
- Demonstrated ability to support and mentor attendant care workers and nursing staff. This includes assessing the competence of attendant care workers and nurses including the reporting of progress / issues and delivering timely and accurate feedback.
- Demonstrated effective written and verbal communication skills
- Demonstrated ability to consult, negotiate and collaborate with internal and external key stakeholders, clients and colleagues within the multidisciplinary environment to cultivate productive working relationships and to support best health outcomes for clients
- A commitment to WH&S
- Available for occasional relief in After Hours on-call roster
- Drivers license & vehicle
- All applicants must hold or be willing to obtain a current Working with Children Check, NDIS Workers Screening Check and National Police Check.

**Job Types**: Full-time, Permanent

Pay: $39.33 - $40.66 per hour
