**About the Role**:
Ensure rostering and scheduling of Services is logístically sound efficient and effectively utilises staffing resources.
- Ensure compliance with the referral requests to appropriately allocate staff to clients based on compatibility.
- Provide mentoring, support, and guidance to the Home Care Workers through supervision and consistent communication.
- Coordinate the internal process of service delivery that are accountable, accurate, clear and effective
- Maintain accurate client information records, whilst adhering to the Aged Care Quality Standards.

**About you**:

- Demonstrated equivalent Certificate IV, Diploma, and Degree in Community Services, Health Care (or approved equivalent) and/or relevant work experience
- Demonstrated experience in rostering and scheduling the care coordination in either Aged Care, Disability and or Health Services.
- Demonstrated experience in using a Customer Relationship Management (CRM) System
- Knowledge or Experience of the Commonwealth Home Support Program (CHSP), and or Aged Care Quality Standards.
- Hold a current unrestricted ACT Working with Vulnerable People card and Working with Children Check
- Hold National Police Check,
- Hold Current First Aid and CPR Certificate
- Full Drivers’ License and own vehicle

- Hourly rate of $51.00 - $53.31 (Social, Community, Home Care, Disability Award Level 5), plus superannuation, On Call allowance and Salary Packaging.
