In this key role of **Service Coordinator, **you will work collaboratively with the Aaquila Care, NDIS participants, primary service providers, and other key stakeholders to develop, promote, and maintain the provision of appropriate supports and interventions for people with disability.
Under the direction and guidance of Service and Operations Manager, this role is responsible for the referral generation, intake and on-boarding process for new participants, creating service agreements, and developing and implementing support plans and reviews to ensure responsive service delivery that reflects NDIS practice standards, contemporary service principles and exceptional care.
**Core responsibilities of the Service Coordinator role include**:
- Contributing to the development and growth of Aaquila Care, and identifying and developing ongoing, collaborative relationships with sector and community service networks.
- Overseeing the quality of Aaquila Care support service to individual NDIS participants.
- Consulting with participants, Service and Operations Manager and where appropriate their representative/s, other stakeholders and Aaquila Care in the development of participant service agreements and support plans.
- Promoting evidence-based best practice principles in the assessment, planning, implementation and evaluation of support interventions and quality service delivery for participants.
- Initiating and maintaining productive relationships and strong rapport with participants and their representatives, external stakeholders, and service providers who may present with differing priorities.
- Participating in case conferences and other participant review processes, including the development of progress reports when required.
- Assisting in the investigation and review of incident reports, participant observation records, and the subsequent identification of unmet needs, risks and non-compliance with NDIS practice standards and Code of conduct, legislative frameworks and Just Living policies and procedures
- Be responsible for coordinating and managing accommodation services for Aaquila Care Participants. This includes assisting with visiting the homes, coordinating tenancy agreements, and providing ongoing support to ensure a safe and comfortable living environment.
**Essential requirements of the Service Coordinator role include**:
- Tertiary qualification in social work, human services, community services or other relevant discipline, **or **equivalent relevant professional experience exceeding 3 years.
- Demonstrated knowledge and understanding of NDIS compliance and practice standards, and processes, and other relevant industry standards.
- Sound understanding of, and compliance with NDIA Quality & Safeguarding Commission Reportable Incidents and other reporting requirements.
- Ambitious, growth orientated mindset.
- Demonstrated competency in a similar role, including responsibility for the day-to-day management and oversight of quality service provision in the disability and community service sector, and experience of contemporary service delivery for people with complex needs.
- Demonstrated competency in working within a recognised quality management system; analysing and interpreting information and providing advice and reports that contribute to the overall quality of Just Living service delivery.
- Sound understanding of the client experience in the disability and community sector and current best practice principles.
- Exceptional written, verbal, interpersonal and presentation skills.
- Sound risk identification and mitigation skills.
- Ability to work positively in a team environment and work under pressure.
- Ability to maintain confidentiality and privacy and to work in a position of trust.
- Valid SA driver’s license and access to a reliable, insured motor vehicle.
- To hold and maintain the applicable state clearance to work with children is an essential requirement for this role and continued employment with Just Living.
- To hold and maintain an approved First Aid clearance.
