Western Health is seeking an experienced Registered Nurse to join the dynamic Central Access Unit (CAU) and ACE Inpatient teams, in a dual care coordination role. The CAU and ACE IP Care Coordinator, improves access to WH Chronic and Complex Care services by early identification of patient needs and potential barriers to discharge, completion of timely referrals to both inpatient (iPM) support and post discharge CAU supports.

We are looking for a person who is passionate about integrated care and patient flow to triage, direct, reject or redirect referrals to community programs. The Care Coordinator, when working in the post discharge area, monitors clients by telephone and follows up on any clinical issues should the need arise, for patients post discharge from hospital. The role ensures that patient care is being delivered in accordance with the arranged care plan and provides a single point of contact for the consumer. The role is permanent part-time and will be required to work across a 7-day rotating roster including public holidays.

**To be Successful in this Role you have**:

- a minimum of 5 years post graduate acute and community experience
- highly developed problem solving and negotiation skills
- well-developed interpersonal and communication skills
- ability to work in a faced paced environment in a flexible way
- a comprehensive knowledge of community care options available in the western suburbs
- experience in discharge planning and/or clinical triage would be highly regarded
