**About Your Role**

Scope has an exciting and unique opportunity for a Service Transition Coordinator to work within our Intake Division.
- Work with a collaborative team to advance client, family and employee experiences
- Identify, explore and advocate for opportunities to improve on how we onboard clients
- Develop and maintain respectful, productive and ethical relationships with relevant internal and external stakeholders
- Maintain accurate documentation and communicate with stakeholders to clarify expectations

**What We Look For**
- Ability to collaborate with internal and external stakeholders
- Priortise and manage competing deadlines
- Experience within disability services and/ or related sector
