**Clinical Care Manager - Home Care Packages**

**Job description**

Sensible Care is an approved provider of Home Care Packages and a registered provider of NDIS Disability Support Services. We service all areas of Melbourne and Greater Geelong.

**About the Role**:
**Tasks and duties**
- Provide clinical and technical support to care management team.
- Review and monitor care management documentation, care plans and risk registers ensuring all compliance documentation are accurately maintained at all times.
- Monitor all clinical and regulatory developments in home and community care domain; support, mentor and train the care management team to keep up-to-date with the regulatory framework.
- Develop and/or enhance methods and procedures to improve the quality of care management processes.
- Ensure all care management team members’ clinical and technical skills and knowledge for their positions are current. Identify knowledge gaps and training needs and deliver in-house training to care management team.
- Be committed to participation in continuous improvement activities to ensure that a high-quality clinical management standard is maintained.
- Work with the broader team on quality and continuous improvement initiatives.
- Be a pivotal leader in risk identification and risk management in delivery of care.
- Maintain a reduced case load of Home Care Package clients commensurate with the clinical management responsibilities.
- Taking an active role in building relationships with client groups and referral sources and representing Sensible Care in business development activities.

**Essential key selection criteria**:

- A formal clinical/nursing degree
- Previous experience and demonstrated knowledge in home and community care services
- Time management skills to effectively manage workload and meet deadlines
- A high level of interpersonal skills including the ability to develop and maintain relationships within the team, with clients and participants, across service providers, and other stakeholders
- An Australian Driver’s Licence and own vehicle
- Ability to manage competing priorities
- Strong attention to detail and well organised
- A commitment to providing the best quality support and services to our clients

This role also requires the successful applicant to have or obtain a national police check and submit two referees.

**Job Types**: Full-time, Part-time
Part-time hours: 28-38 per week

**Salary**: $79,634.60 - $114,608.00 per year

- Diploma (preferred)
