**Your new role**
As a Client Care Liaison, you will proactively work to coordinate the in-home care of older Australians with a funded Home Care Package (HCP) through the Australian Department for Health and Ageing.
You will build relationships of trust and cooperation through clear and transparent communication with our clients, working autonomously while supported by a friendly and welcoming team.
You will undertake a range of activities which may include:
- Effectively coordinating client support services by conducting initial assessments and service reviews, developing care plans for clients and the provision of ongoing case management as per program requirements and under supervision of the Home Care Manager
- Preparing individualised budgets from allocated HCP funding in partnership with clients and review as required.
- Monitoring and reviewing client needs and update Agreed Support Plans to reflect any changes.
- Contributing to the collection, input and reporting of activity and service provision data using a variety of data management systems in addition to maintaining a high level of data integrity.
- Attending direct client care (in-home, hospital, residential care) as required.
**As the successful Client Care Liaison, you will possess**:
- Current AHPRA Registration
- Current DHS Clearance (WWCC, Aged, Disability/NDIS)
- Current Manual Handling, First Aid, CPR and Basic Life Support Certificates
- National Police Clearance / NPC
- Full / Unrestricted South Australian Drivers License
